Submitting a Sample to QC Laboratories

Samples may be submitted to QC Laboratories directly by the client, by one of QC's field representatives (who may have collected samples for you), or by any common carrier. If you require shipping containers for your samples, simply contact one of our client service representatives to arrange for delivery of QC's new Sample Paks. Our Sample Paks are complete with sample containers, ice packs, and packaging materials inside a foam-lined shipping carton (see the end of this section for an illustration). Sample request for analysis forms, chain of custody record forms, and correspondence are also included in each Sample Pak.

If you choose to send a sample without a SamplePak, please submit the following information with your samples:

• Your name or the name of your company, along with the complete address to which the final report should be sent.
• A complete billing address.
• A full description of the samples submitted, including collection date(s) and an identification label unique to each sample.
• A completed Chain of Custody form (click here for download [link to download of Chain of Custody email) with signatures, dates and times for each transfer of sample custody, and a list of analyses requested.
